Step 1: Click “Get Started”
Step 2: Click “Medicaid card”
Step 3: Enter Provider One card number
Step 4: Select “Washington” in drop down menu
Step 5: Upload picture of Provider One card
Step 6: Put check in box “I confirm my EBT card is current and valid and to enable the continue button”
Step 7: Click “Save 50% of Prime” button

and uploaded the Provider One card.
